Unity Health provides easy access to request your medical records electronically through our partnership with Datavant. Datavant is a trusted and secure portal allowing patients to request all or part of their medical records. Medical records can be delivered electronically or through the US Postal Service. You will be required to capture or upload a photo ID as part of the request. Click the button below to begin.
The request will be verified, and you will be contacted if there are any questions or concerns. An email notification will be sent confirming the request is complete.
